Care that meets people where they live.
The Harvest Community Group connects Inland Empire residents to care, housing support, and community resources, starting in Perris and growing across Riverside County.
Every person deserves an opportunity to achieve stability, health, and independence.
Our mission is to serve individuals and families who need the greatest level of support by helping them access healthcare, housing, employment resources, mental health support, and community services.
Our long-term vision is to become a comprehensive wraparound services organization serving Riverside County and surrounding communities.
Our current programs
Practical, person-centered support for residents who need help coordinating care and finding stable ground.
Enhanced Care Management (ECM)
A dedicated care manager helps eligible Medi-Cal members coordinate their health care, social services, and community supports in one plan.
Community Supports
Services that address needs beyond the clinic, such as housing and other non-medical help, available through Medi-Cal health plans.
Housing navigation
Hands-on help to understand housing options, prepare applications, and take the next step toward a stable home.
Community resources
Connections to local organizations and services that support food, health, and daily living needs.
Eligibility for ECM and Community Supports is determined through each member's Medi-Cal health plan. If you are unsure whether you qualify, contact us and we will help you find out.
Stable housing is the foundation of health.
Today, we help residents navigate housing options and connect to housing-related Community Supports. Our long-term vision includes affordable and permanent housing for Inland Empire families.
