Our current programs
Enhanced Care Management (ECM)
A dedicated care manager works with you to bring your health care, social services, and community supports together in one plan.
ECM is a Medi-Cal benefit for eligible members, provided through their Medi-Cal health plan. Your care manager helps you understand your options, keep appointments, and stay connected to the people and services that support you.
Community Supports
Services that address needs outside the doctor's office, such as housing, so that people can stay healthy and stable.
Community Supports are offered through Medi-Cal health plans. We help people understand what may be available to them and how to request it.
Housing navigation
Hands-on help to understand housing options and take the next step toward a stable home.
We help people learn what housing resources exist, prepare for applications, and keep moving forward.
Community resources
Connections to local organizations and services that support food, health, and daily living needs.
If we cannot meet a need ourselves, we work to point you to someone who can.
Eligibility for ECM and Community Supports is determined through each member's Medi-Cal health plan. If you are unsure whether you qualify, contact us and we will help you find out.

Where we are headed
We are working toward affordable and permanent housing, behavioral health and rehabilitation services, substance use recovery programs, reentry services, and employment opportunities. These are goals for our future growth, not current services.
